Birth and death certificates are available from the Colorado Department of Public Health and Environment (CDPHE).

Request Your Medical Record

Patients may request a copy of their Rose Medical Center medical record from our Health Information Management office. In order to request your medical record, download and complete a medical records release form and submit the completed form via mail, fax or in person. Records are made available within 10 days of your request.

If you cannot download the above form, please submit a written request to the above address or fax number that includes the patient's name, date of birth, social security number, requested information, dates of service, and it must be signed (by patient) and dated.
