Baby's Birth Certificate and Social Security Number

After delivery, you will be asked to provide information for your baby's birth certificate. The birth certificate is an important, permanent legal document that will be used by your child throughout life. State law requires that a Certificate of Birth be filed with the Colorado State Registrar within 10 days after birth, so it is helpful if you decide on a name and spelling before we request this information.

If you are not married, but both you and the father of your baby would like to have the father's name and information added to your baby's birth certificate, this can be done by signing a paternity form. Both you and the father need to sign the paternity form and have your signatures witnessed by hospital personnel. Without this signed paternity form, the father's information cannot be added to the Birth Certificate. Please call our Medical Records Department at 303-320-2663 with any questions.

After completion, the hospital will send the Birth Certificate to the State Registrar's Office, where your baby's birth will be legally registered. If changes or corrections have to be made after the State Registrar has processed the certificate, there will be a fee. Later changes may require a court order.

While in the hospital, you may begin the process of obtaining a Social Security number for your child through the state offices of Vital Records and Social Security. The card arrives approximately 12 weeks after filing.

Baby's Picture

Your baby's first picture is an irreplaceable keepsake. You will receive information about baby pictures when you arrive on the Mother/Baby unit. On site are photographers from Pitter Patter Pix that take digital newborn pictures upon your request. These can be put on a Web site or made into prints, or viewed online through their website. To choose this service, please sign the consent form in your folder early in your stay to allow the photographer to contact you for an appointment.
