Diabetes Education
Our Diabetes Education Program includes the three classes outlined below, as well as two follow-up session. This program teaches patients all aspects of diabetes health, goal setting and behavior change.
- Assessment and Behavior Change
- This course is three hours, offered two to three times a week. Topics include diabetes assessment, stages of change, and support systems that can help maintain health.
- Core Curriculum
- This course is four hours and is offered four to five times every month. Topics include proper nutrition, blood sugar monitoring, complication prevention, exercise enhancement and overall diabetes health.
- Meal Planning
- This course is two hours and is offered two to three times a week. Topics include weekly meal plans, eating out and reading food labels.
- Three-Month Follow-Up
- This course is one hour and is offered two times a month. Topics include revisiting initial behavior change goals and maintenance of diabetes health.
- Twelve-Month Follow-Up
- This course is one hour and is offered two times a month. Topics include re-visiting goals, discussing future diabetes challenges and accessing community resources.
